Efficiency of glucocorticosteroids in severe alcoholic hepatitis

Introduction . The feasibility and risks of glucocorticosteroids (GCS) in severe alcoholic hepatitis (SAH) are actively discussed, there is a real need to develop new biomarkers both determine indications for the GCS use evaluate their effectiveness. Аim Тo effectiveness SAH using marker hepatocyte apoptosis inflammatory cytokines along with traditional laboratory parameters. Materials methods Prednisolone at dose 40 mg per day was received by 68 patients SAH. therapy assessed after 7 days Lille index, level cytokeratin-18 fragments (FCK-18) – IL-1β, TNF-α, IL-6 IL-8. Results A positive effect noted 50 (73.5%) patients, index 0.23 ± 0.09, levels FCK18, IL-6, IL-8, TNF-α were significantly reduced, subsequent decrease improvement hepatic functional These had 100% short-term (within 28 days) survival rate. Eighteen (26.5%) negative result, 0.61 0.11, no significant FCK-18 cytokines. After discontinued, they developed liver failure, 1/3 bacterial infections, all died multiple organ failure within days. Conclusion 73.5%. Along following indicators demonstrated diagnostic significance: cytokeratin-18, and, lesser extent, IL-1β.
